目的:研究APRI指数对乙肝患者肝脏纤维化程度的评估价值及其与FibroScan检查的相关性。方法:选择慢性乙型肝炎伴肝纤维化的病例作为研究的病理组、健康志愿者作为研究的对照组,检测血清中天门冬氨酸氨基转移酶(AST)和血小板(PLT)含量并计算APRI指数,采用FibroScan测定肝脏弹性测量值(LSM),采用荧光定量PCR方法测定TGFβ1、Smad2、Smad3的mRNA含量。结果:病理组的APRI指数、LSM均高于对照组,不同纤维化程度患者的APRI指数和LSM有差异且纤维化分级越高、APRI指数和LSM也越高;不同纤维化程度肝脏活检组织中TGFβ1、Smad2、Smad3的mRNA含量有差异,纤维化分级越高,肝脏活检组织中TGFβ1、Smad2、Smad3的mRNA越高;APRI指数与LSM以及肝脏活检组织中TGFβ1、Smad2、Smad3的mRNA含量呈正相关。结论:APRI指数能够准确评估乙肝患者肝脏纤维化程度,APRI指数与FibroScan测得的LSM以及肝纤维化病理进程具有良好的相关性。
Abstract：Objective:To study the value of APRI index in evaluation of liver fibrosis degree of patients with hepatitis B and its correlation with FibroScan check.Methods:Chronic hepatitis B patients with liver fibrosis were selected as pathology group and healthy volunteers were selected as control group.Serum aspartate aminotransferase(AST)and platelet(PLT)contents were detected and APRI index was calculated;liver stiffness measurement(LSM)was measured by FibroScan;mRNA levels of TGFβ1,Smad2 and Smad3were measured by fluorescent quantitative PCR method.Results:APRI index and LSM of pathology group were higher than those of control group,APRI index and LSM of patients with different degrees of fibrosis were different and the higher the fibrosis grading,the higher the APRI index and LSM;mRNA contents of TGFβ1,Smad2 and Smad3in liver biopsy tissue with different degrees of fibrosis were different and the higher the fibrosis grading,the higher the mRNA contents of TGFβ1,Smad2 and Smad3in liver biopsy tissue;APRI index was positively correlated with LSM and mRNA contents of TGFβ1,Smad2 and Smad3in liver biopsy tissue.Conclusion:APRI index can accurately assess the degree of liver fibrosis in patients with hepatitis B,and APRI index has good correlation with the LSM measured by FibroScan and the pathological process of liver fibrosis.
