目的:探讨核基质蛋白(NMP22)、膀胱肿瘤抗原(BTAstat)和B超联合检查在膀胱肿瘤诊断和术后随访中的价值。方法:对90例怀疑膀胱肿瘤以及膀胱癌术后随访的患者,均在预约膀胱镜检查前行尿NMP22、BTAstat和经腹B超检查,以病理检查为金标准,分析评价NMP22、BTAstat和B超联合检查在膀胱肿瘤诊断中的作用。结果:90例中膀胱移行细胞癌患者41例,泌尿系统其他疾病患者49例,NMP22、BTAstat和经腹B超检查3者联合诊断膀胱肿瘤的敏感性为92.7%,与膀胱镜(92.7%)比较,差异无统计学意义(P=1),联合检测膀胱肿瘤的特异性、阳性预测值、阴性预测值分别为75.5%、79.1%、91.2%。结论:NMP22、BTAstat和B超联合检查有助于提高膀胱肿瘤早期诊断的能力,可部分替代膀胱镜检。
Abstract：Objective: To assess the value of combining use of NMP22,BTAstat and ultrasonography in Screening and Surveillance of bladder tumor(BT).Methods: NMP22,BTAstat and ultrasonography were carried out for 90 symptomatic patients suspicious of BT or after surgical treatment before cystoscopy.Pathology was used as the gold standard.Analysis the sensitivity(Se),specificity(Sp),positive perdictive value(PPV) and negative perdictive value(NPV) for each and conbined using in bladder tumor.Results: 41 bladder transitional cell carcinomas were diagnosed by pathology.49 cases were other benign urological diseases.With conbined use of 3,the Se(92.7%)was equal to the cystoscopy(92.7%)(P=1).The cinbined Sp,PPV,NPV was 75.5%79.1%91.2%respectly.Conclusions: Conbined use of NMP22,BTAstat and ultrasonography improve the diagnostic value in early BT.It can be adopted of cystoscopy partly.
