目的:探讨腹腔镜下保守性手术治疗输卵管妊娠的手术方式、疗效及预后。方法:回顾分析2006年1月～2008年7月经腹腔镜保守性手术配合甲氨喋呤治疗的60例(腹腔镜组)输卵管妊娠的患者,并与同期剖腹行输卵管妊娠保守性手术60例(对照组)作对比研究。结果:与对照组相比,腹腔镜组手术时间、住院时间、术后肛门排气时间明显缩短(P均<0.05),术中出血量明显减少(P<0.05)。腹腔镜组输卵管复通率(96.8%,31/32)、术后受孕率(46.7%,28/60)均明显高于对照组(64.5%,20/31;20.0%,12/60)(P均<0.05)。结论:腹腔镜下保守性手术治疗输卵管妊娠疗效满意,术后宫内妊娠机率优于剖腹组,此术式是未育患者的较佳选择。

Objective:To explore the operation method,effect and prognosis of conservative operation treated tubal pregnancy by laparoscope.Methods:Data during January 2006 and July 2008 were retrospectively analyzed.Conservative surgery treatment by laparoscopic with methotrexate were selected in 60 tubal pregnancy patients(laparoscopic group).And 60 tubal pregnancy patients who were selected cesarean and conservative operation were compared synchronously(control group).Results:The operation time,length of stay and flatus time after operation,after operation were significant lower when compared to the control group(Pall<0.05).The intraoperative bleeding was less(P<0.05).The repatency rate of salpinxes was 96.8% and the intrauterine pregnancy rate was 46.7% in laparoscope group,which were all significantly higher than that of control group(64.5% and 20.0% respectively)(P<0.05).Conclusions:Conservative operation treating tubal pregnancy by laparoscope could get satisfactory effect and postoperative intrauterine pregnancy rates were better than those who get cesarean.The operation could be made asthe first choice in patients without offsprings.
