目的:探讨牙周的基础治疗对牙周病患者正畸治疗的效果的影响。方法:纳入本院牙周病患者30例,于治疗前T0及3个月单纯牙周治疗后T1及联合牙周正畸治疗后T2,测定患者的牙周健康指标(牙周袋探诊深度PD、松动度TM、探诊出血BOP和临床附着丧失CAL)的检查及拍摄CBCT测定牙槽嵴顶到釉牙骨质界的距离、牙根长度及牙槽骨密度的变化。结果:T1松动度明显少于T0,差异有统计学意义(P<0.05),T2的松动度有所增加;T1探诊出血点的百分比明显少于T0,T2又小于T1,差异有统计学意义(P<0.05);牙周袋的探诊深度T1明显少于T0,T2又小于T1,差异有统计学意义(P<0.05);临床附着丧失T1明显少于T0,但是T2与T1相比,差异无统计学意义(P>0.05);牙根长度T2与T1相比没有统计学差异,牙周正畸联合后并未引起牙根吸收增加,差异无统计学意义(P>0.05);牙槽嵴顶到釉牙骨质界的距离牙周正畸联合后并未引起牙槽骨吸收,差异无统计学意义(P>0.05);牙槽骨密度的变化T1优于T0,牙周正畸联合后牙槽骨密度反而减少了。结论:牙周病患者正畸治疗前进行牙周基础治疗可以有效控制牙周病的发展,同时也为进一步正畸治疗改善美观和咀嚼功能提供良好的基础。
Abstract:Objective:To investigate the periodontal therapy on the basis of results of orthodontic treatment of periodontal disease.Methods:Our hospital from January 2013 to November 2014 30 patients with periodontal disease,the treatment after 3months before T0 and after periodontal therapy alone or combined periodontal orthodontic treatment T1T2,measured in patients with periodontal health indicators(probing pocket depth PD,loose degree TM,bleeding on probing,and clinical attachment loss BOP CAL)CBCT examination and determination of alveolar crest shooting distance to CEJ,the root length and alveolar bone density changes.Results:T1loose degree significantly less than T0(P<0.05),increased mobility of T2;T1bleeding on probing point percentage was significantly less than T0,T2 has less than T1(P<0.05);periodontal pocket probe T1 probing depth was less than T0,T2 has less than T1(P<0.05);clinical attachment loss was significantly less than T1T0,T2 compared with T1 but no significant difference(P> 0.05);T2compared with T1 root length no statistically significant difference,did not cause periodontal after orthodontic root resorption increases(P>0.05);alveolar crest to the distance CEJ periodontal orthodontic post did not cause alveolar bone loss(P > 0.05);changes in alveolar bone density than T1T0,the combined orthodontic periodontal alveolar bone density has decreased.Conclusion:Patients with periodontal therapy periodontal orthodontic treatment can effectively control the development of periodontal disease,but also for further orthodontic treatment to improve the appearance and masticatory function provides a good foundation.
