益气活血中药干预OPN对心梗后心衰大鼠心室重构的影响研究

目的:探讨益气活血中药影响心肌梗死后心力衰竭大鼠心肌组织中骨桥蛋白(osteopontin,OPN)水平,改善心室重构的机制研究。方法:准备SPF级SD雄性大鼠40只,保留10只作为空白对照,其余大鼠采用冠状动脉前降支结扎结合减食及力竭式游泳法制备心梗后心衰大鼠模型。空白组和模型组大鼠每日予蒸馏水灌胃,中药组予益气活血中药煎剂灌胃,西药组予卡托普利灌胃。用药6周后应用小动物超声检测大鼠心脏结构及功能变化。处死所有大鼠,取心肌组织,光镜下观察心肌组织形态学变化。应用实时荧光定量PCR法检测大鼠心肌组织中OPN mRNA的表达情况,同时应用免疫组化法检测心肌组织中OPN蛋白的表达情况。结果:与空白组对比,模型组左心室射血分数(EF)、左心室短轴缩短分数(FS)明显降低,左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)明显升高,OPN mRNA及蛋白表达均明显上调(P<0.01),光镜下可见心肌结构发生紊乱。与模型组对比,中药组与西药组EF、FS明显升高,LVEDD、LVESD明显降低,OPN mRNA及蛋白表达均明显下调(P<0.01),光镜下可见心肌结构明显改善。且西药组与中药组疗效相比,差异无统计学意义(P>0.05)。结论:益气活血中药可能通过降低心肌组织中OPN表达水平,改善心室重构,起到提高心功能、防治心肌梗死后心力衰竭的作用。

Abstract:

Objective: :To investigate the mechanism how YiqiHuoxue traditional Chinese decoction affectsthe osteopontin(OPN) level in myocardial tissue and improve theventricular remodeling in heart failure rats after myocardial infarction. Methods: Thirtyty SPF-grade SD male rats were used to prepare models of heart failure after myocardial infarction by anterior descending coronary artery ligation combined with reduced diet and exhausted swimming. The remaining 10 rats in the blank group served as control group. Both the control and the model groups were orally administered with distilled water. The Chinese medicine group was administered with YiqiHuoxue Chinese medicine decoction, and the western medicine group was administered with captopril. After 6 weeks of treatment, small animal ultrasound was used to detect changes in ventricular structure and function in rats. After all the rats were sacrificed, myocardial tissues were taken out to observethe morphological changes of myocardial tissueunder a light microscope. Real-time quantitative PCR was applied to detect the expression of OPN mRNA in rat myocardial tissue, and immunohistochemical method was used to detect the expression of OPN protein in myocardial tissue. Results: Compared with the controlgroup, the left ventricular ejection fraction(EF), left ventricular short axis shortening fraction(FS) ofthe model group were significantly reduced, and the left ventricular end-diastolic diameter(LVEDD) whileleft ventricular end-systolic diameter(LVESD) were significantly increased, OPN mRNA and protein expression were significantly up-regulated(P<0.01), and myocardial structure disorder was observed under light microscope. Compared with the model group, the EF and FS of the Chinese medicine group and the Western medicine group were both significantly increased, the LVEDD and LVESD were significantly reduced, the expressions of OPN mRNA and protein were significantly reduced(P<0.01), and the myocardial structure was significantly improved under light microscopy. There was no statistically significant difference between the western medicine group and the traditional Chinese medicine group(P> 0.05). Conclusions: YiqiHuoxue Chinese medicine may improve ventricular remodeling, enhance cardiac function and prevent heart failure after myocardial infarction by reducing the expression level of OPN in myocardial tissue.
