目的:探讨Gamma钉内固定治疗股骨粗隆间骨折的疗效。方法:自2000年1月~2006年3月收治股骨粗隆间骨折51例,按Evan分型,Ⅱ型和部分ⅢA型采用闭合复位,Gamma钉内固定;其余为切开复位,Gamma钉内固定。结果:随访8~36个月,术后早期并发切口血肿1例、下肢深静脉栓塞2例,无骨折不愈合、髋内翻、内固定松动和断裂,根据X线片和按Harris评分标准,优良率为82.4%。结论:采用闭合复位和切开复位相结合,Gamma钉内固定治疗股骨粗隆间骨折,早期被动和主动功能锻炼,取得较好的临床疗效。

Objective: To investigate the therapeutic effects of internal fixation of Gamma nail for intertrochanter fracture of femur.Methods: 51 cases of intertrochanter fracture of femur were divided into various types according to Evan standard.All types Ⅱ and part of type Ⅲ were treated occlusion reduction with internal fixation of Gamma nail,and others open reduction with internal fixation of Gamma nail.Results: All cases showed union and there were complications including 1 case of incision hematoma,2 cases of deep vein embolism of lawer limb no case of disunion hip inversion,relaxation or fragmentation of internal fixation after 8～36 months fallow up.The fine rate was 82.4% according to X ray film and Harris estimation standard,Conclusion: The internal fixation of Gamma nail for intertrochanter fracture of femur can get better therapeutic effects,when combined occlusion reduction and open reduction.
