目的:探讨化银解毒汤治疗脓疱型银屑病的作用机制。方法:将60例银屑病患者随机分为观察组和对照组两组,正常对照组系正常体检健康人30例。两组均治疗8周,观察组口服化银解毒汤,对照组口服阿维A治疗,在健康组体检前及两组治疗前后4、8周抽血检验转化生长因子-β1(TGF-β1)、基质金属蛋白酶-9(MMP-9)、基质金属蛋白酶组织抑制剂-1(TIMP-1),并讨论之间的相关性。结果 :两组治疗前后PASI评分明显下降,差异具有统计学意义(P<0.01);治疗两组治疗前和健康对照组比较,TGF-β1、MMP-9、TIMP-1的血清水平均明显增高,差异具有统计学意义(P<0.01);两组治疗后血清中TGF-β1、MMP-9、TIMP-1的水平较治疗前明显下降,差异具有统计学意义(P<0.01);两组比较,观察组在临床疗效,降低患者PASI积分,降低血清TGF-β1、MMP-9、TIMP-1方面均较优,尤其在8周更明显,差异有统计学意义(P<0.01)。结论:银解毒汤通过影响TGF-β1、MMP-9、TIMP-1来发挥治疗银屑病的作用,疗效优于阿维A。
Abstract:Objective:To study the effect of Huayinjiedu decoction on pustular psoriasis.Methods:By randomized controlled methods,60 patients with psoriasis were randomly divided into two groups,the observation group and the control group.Another 30 health persons were selected as normal control group.The course of treatment was 8weeks.Patients in observation group were treated by oral administration of Huayinjiedu decoction,while patients in the control group were treated by oral administration of acitretin.TGF-β1,MMP-9and TIMP-1were detected before and 4,8weeks after treatment in observation group and control group;while these indexes were detected before physical examination in normal control group.Results:After treatment,PASI score was significantly decreased in observation group and control group(P< 0.01).Before treatment,compared with healthy controls,TGF-β1,MMP-9and TIMP-1levels in observation group and control group were significantly increased(P< 0.01).After treatment serum TGF-β1 MMP-9and TIMP-1levels were decreased significantly in observation group and control group(P < 0.01).The decreases in curative effect,PASI score,serum TGF-β1,MMP-9and TIMP-1were more significant in observation group,especially in the 8th weeks(P< 0.01).Conclusions:Huayinjiedu decoction has significant effect on TGF-β1,MMP-9and TIMP-1.Its curative effect is superior than acitretin.
