目的:观察蛛网膜下腔注射舒芬太尼及舒芬太尼复合罗哌卡因在腰硬联合阻滞分娩中的镇痛效果。方法:选择初产妇60例,随机分为A、B两组,每组30例。A组蛛网膜下腔注射舒芬太尼5μg、罗哌卡因3 mg,B组蛛网膜下腔注射舒芬太尼5μg。当产妇VAS≥3分时启用自控PCA泵维持镇痛,并在不同时间点对两组进行VAS评分,记录镇痛维持时间、不良反应及运动阻滞情况等指标。结果:A组镇痛起效时间、镇痛效果、镇痛维持时间均明显优于B组(P<0.05或0.01),并且对产程、新生儿Apgar评分的影响较小。结论:舒芬太尼复合罗哌卡因行腰硬联合阻滞分娩镇痛效果良好,具有起效快、镇痛强、持续时间长、副作用少等优点。

Objective:To investigate the analgesia effect of combination of sulfentanyl and ropivacaine on combined spinal and epidural anesthesia for delivery.Methods:A total of 60 primiparas were selected,and randomly divided into two groups by half.Group A was treated with cavitas subarachnoidealis injection of sulfentanyl 5 μg and ropivacaine 3 mg;while Group B with sulfentanyl 5 μg only.Automatic PCA pump was used for sustaining analgesia as VAS≥3.VAS scale was also performed at different time,and some indexes including maintenance time,status of side effect and motor block.etc.were recorded.Results:Group A showed shorter onset time of analgesia,better analgesia effect and longer maintenance time(P<0.05 or 0.01),and less effect on delivery course and infantile Apgar score.Conclusions:Combination of sulfentanyl and ropivacaine is of shorter onset time of analgesia,better analgesia effect,longer maintenance time and less side effect.It is valuable in combined spinal and epidural anesthesia for delivery.
