目的:研究重症急性胰腺炎患者体内肾素-血管紧张素-醛固酮(RAAS)系统活性与全身炎症反应及靶器官功能的关系。方法:选择在本院就诊的急性胰腺炎患者并分为MAP组和SAP组,另取同期体检且一般资料匹配的健康志愿者作为对照组。采集血清并检测RAAS系统分子、炎症反应分子、肝功能及肠黏膜屏障功能损伤分子的含量。结果:SAP组和MAP组患者血清中PRA、AngII、ALD、TNF-α、sTREM-1、PCT、CRP、LPS、DAO、HBD2的含量均显著高于对照组;SAP组患者血清中PRA、AngII、ALD、TNF-α、sTREM-1、PCT、CRP、LPS、DAO、HBD2的含量均显著高于MAP组;SAP组患者血清中PRA、AngII、ALD的含量与TNF-α、sTREM-1、PCT、CRP、LPS、DAO、HBD2的含量呈正相关。结论:重症急性胰腺炎患者体内RAAS系统的激活与全身炎症反应级联放大、靶器官功能损伤密切相关。
Abstract:Objective:To study the relationship of renin-angiotensin-aldosterone system(RAAS)activity with systemic inflammatory response and target organ function in patients with severe acute pancreatitis.Methods:The patients with acute pancreatitis admitted in Zigong Third People's Hospital between June 2014 and December 2016 were selected and divided into MAP group and SAP group,and the healthy volunteers who received physical examination during the same period and were with matched general data were selected as control group.Serum was collected to determine the levels of RAAS molecules,inflammation molecules as well as liver and intestinal mucosal barrier injury molecules.Results:Serum PRA,AngII,ALD,TNF-α,sTREM-1,PCT,CRP,LPS,DAO and HBD2 contents of SAP group and MAP group were significantly higher than those of control group;serum PRA,AngII,ALD,TNF-α,sTREM-1,PCT,CRP,LPS,DAO and HBD2 contents of SAP group were significantly higher than those of MAP group;serum PRA,AngII and ALD contents of SAP group were positively correlated with TNF-α,sTREM-1,PCT,CRP,LPS,DAO and HBD2 contents.Conclusions:The activation of RAAS system in patients with severe acute pancreatitis is closely related to the amplification of systemic inflammatory response and the damage of target organs.
