目的：探究湿疹患者血清总IgE水平和外周血EOS计数与临床上湿疹严重程度的相关性。方法：利用EASI和瘙痒VAS评分法评估100例湿疹患者的疾病严重程度，根据EASI评分将湿疹患者分为轻度（<7分）、中度（7～15分）、重度（>15分）3组。同时在4周的中药治疗前后各进行一次血清总IgE水平和外周血EOS计数检测。结果：41%的湿疹患者血清总IgE水平升高，26%外周血EOS计数增多；在轻度、中度、重度3个组别中，随着湿疹严重程度的增加，血清总IgE水平、外周血EOS计数不断增长，阳性率明显提高（P均<0.05）；相关性分析方面，研究显示湿疹患者总IgE水平与EASI评分（r=0.421,P<0.01）、EOS计数与EASI评分（r=0.326,P<0.01）、EOS计数与瘙痒VAS评分（r=0.529,P<0.01）均呈相对较弱的正相关关系；而高比例的患者经过治疗后总IgE水平、EOS计数的变化与EASI变化情况一致（IgE与EASI一致者占82%,EOS与EASI一致者占76%）。结论：本研究显示湿疹患者血清总IgE水平、外周血EOS计数与湿疹严重程度呈正相关，但同时此种相关性较为有限，临床上需同时结合个体自身指标的动态变化，才能为湿疹的诊断、综合治疗及疗效评价等方面提供更大的参考价值。

Objective:To explore the correlation between the levels of total serum immunoglobulin E(IgE),peripheral blood eosinophil(EOS)counts and the severity of eczema. Methods:The EASI and pruritus VAS scores were used to evaluate the disease severity of 100 patients with eczema. According to the EASI score,patients with eczema were divided into three groups:mild(<7 points)、moderate(7-15 points)and severe(>15 points). At the same time,the serum total IgE levels and peripheral blood EOS counts were tested before and 4 week after Chinese medicine treatment. Results:Elevated total serum IgE levels were seen in 41% of the patients,and increased peripheral blood EOS counts in 26%. In the three groups of mild,moderate,and severe,as the severity of eczema increased,the levels of total serum IgE and peripheral blood EOS counts continued to increase,and the positive rate increased significantly(all P<0.05). In terms of correlation analysis,the study showed that in patients with eczema total IgE levels and EASI scores(r=0.421,P<0.01),EOS counts and EASI scores(r=0.326,P<0.01),EOS counts and pruritus VAS scores(r=0.529,P<0.01)all showed a weak positive correlation;and a high proportion of patients had the same changes in IgE and EOS after treatment(82% of IgE is consistent with EASI,76% of EOS is consistent with EASI).Conclusion:Total IgE levels and EOS counts of patients with eczema are positively correlated with the severity of eczema,but at the same time this correlation is relatively limited. In clinical practice,it is necessary to combine the dynamic changes of the individual's own indicators in order to diagnose,comprehensively treat and evaluate eczema. Providing greater reference value in other aspects.
