单纯性肥胖儿童血清瘦素、胰岛素抵抗、内分泌激素及血脂与生长发育的关系研究

摘要:

目的:分析单纯性肥胖儿童血清瘦素、胰岛素抵抗、内分泌激素及血脂水平与其生长发育的相关性。方法:选择200例单纯性肥胖儿童(研究组)和同期200例非肥胖儿童(对照组)进行研究,检测空腹血糖(FBG)、糖化血红蛋白(HbAlc)、血清瘦素(Leptin)、胰岛素(INS)、胰岛素抵抗指数(HOMA2-IR)、生长激素(GH)、皮质醇(cortisol)、甲状腺激素T3及血脂各指标并进行对比分析。结果:研究组Leptin、cortisol及T3水平均较对照组明显增加,而GH水平较对照组明显减少,差异均有统计学意义(P<0.05);研究组HbAlc、INS和HOMA2-IR水平均较对照组明显增加,差异均有统计学意义(P<0.05);而两组FBG水平比较,差异无统计学意义(P>0.05);研究组TC、TG和LDL-C均较对照组明显增加,而HLD-C水平较对照组明显减少,差异均有统计学意义(P<0.05)。结论:单纯性肥胖儿童存在瘦素抵抗、生长激素分泌减少及内分泌和血脂代谢紊乱等,且严重影响儿童的正常生长发育。

Abstract:

Objective:To analyze the correlation of the serum leptin,insulin resistance,Endocrine hormone,blood lipid and of simple obesity children and growth.Methods:A total of 200 cases of simple obesity children(Study Group)and 200 cases of non obese children(control group)were selected.Fasting blood glucose(FBG),Glycosylated hemoglobin(HbAlc),the serum leptin detection(Leptin),insulin(INS),insulin resistance index(HOMA2-IR),growth hormone(GH),cortisol(cortisol),thyroid hormones and blood lipid indexes were detected.Results:Leptin,cortisol and T3 levels of study group were significantly increased compared with the control group,and GH level was significantly reduced(P<0.05).FBG levels showed no significant difference between two groups(P> 0.05),HbAlc,INS and HOMA2-IR were significantly higher than the these of the control group(P< 0.05).TC,TG and LDL-C of study group were significantly higher than those in the control group,while HLD-C was significantly lower(P<0.05).Conclusion:Children with simple obesity show leptin resistance,reduction of growth hormone and endocrine and lipid metabolism disorder,which seriously affect the normal growth and development of children.
