目的:探讨颅内动脉瘤破裂并颅内血肿的诊断和手术治疗经验。方法:回顾性分析48例颅内动脉瘤破裂并颅内血肿患者诊断和手术治疗的临床资料。结果:DSA可早期明确诊断颅内动脉瘤伴颅内血肿;48例患者53个动脉瘤颈均夹闭成功,3个月后随访时,GOS评分恢复良好者32例,中残10例,重残3例,植物生存2例,死亡1例。结论:DSA可作为该类患者术前首选检查手段,明确诊断后积极清除颅内血肿的同时夹闭动脉瘤能有效改善患者预后。
Abstract：Objective: To discuss the diagnosis and experience in microsurgery of ruptured intracranial aneursym complicated with intracranial hematomas.Methods:Clinical data of 48 patients with ruptured intracranial aneurysms complicated with intracranial hematoma were retrospectively analyzed.Results:Ruptured intracranial aneurysms complicated with intracranial hematoma can be easily diagnosed according to DSA.Fifty three aneurysms in 48 patients were clipped.Follow up after 3 months showed good outcomes in 32 cases,moderate disability in 10 cases,severe disability in 3 cases,vegetative state in 2 cases and death in 1 according to GOS scores.Conclusions:DSA is essential for the corrected diagnosis of intracranial hematoma due to ruptured intracranial aneurysm,clearance of hematoma and aneurysm clipping can improve patients' prognosis.
