目的:探讨阿加曲班对急性缺血性脑卒中患者的临床疗效及对血小板功能的影响。方法:将我院收治的85例缺血性脑卒中患者随机分为观察组(43例)和对照组(42例),对照组口服硫酸氢氯吡格雷片,观察组静脉滴注阿加曲班,对比两组患者治疗后脑神经功能缺损、日常行为能力评分变化,并对患者血小板功能进行分析。结果:观察组治疗2、4周后MESSS评分显著低于治疗前,ADL评分明显高于对照组(P<0.05);对照组治疗4周后MESSS评分显著低于治疗前,ADL评分明显高于治疗前(P<0.05);观察组治疗后各时段MESSS、ADL评分显著优于对照组,两组间相比差异具有统计学意义(P<0.05);观察组治疗1周后血小板聚集率(PA)显著高于对照组(P<0.05);治疗前后两组患者血小板计数、部分凝血酶原时间(APTT)差异无统计学意义(P>0.05)。结论:阿加曲班能够改善急性缺血性脑卒中患者脑神经功能,提高患者的日常生活能力,值得临床推广。

Objective:To explore the effect of argatroban on platelet function in acute ischemic stroke patients.Methods:A total of 85 cases with ischemic cerebral stroke were randomly divided into the observation group(43cases)and control group(42cases).Patients in the control group were treated by oral Clopidogrel Bisulfate Tablets,patients in observation group with intravenous argatroban.Activities of daily living,cranial nerve function defect score,and the platelet function were compared.Results:MESSS scores in observation group on the 2nd weeks,4th weeks were significantly lower than that before treatment,ADL score was significantly higher than that of the control group(P<0.05);in the control group after 4weeks of treatment in patients with MESSS scores were significantly lower than that before treatment,ADL score was significantly higher than that before treatment(P<0.05);in the observation group MESSS,ADL of each period scores were significantly better than the control group after treatment,the difference was statistically significant between the two groups(P<0.05);in the observation group PA was significantly higher than that in control group 1week after treatment(P<0.05);but there was no significant difference in platelet count,APTT in two groups before and after treatment(P>0.05).Conclusion:Argatroban can improve brain function of patients with acute ischemic stroke,and can improve the ability of daily life.It is worthy clinical promotion.
