目的:探讨前列地尔联合百令胶囊对早期糖尿病肾病患者肾功能和血清炎性因子的影响。方法:将84例早期糖尿病肾病患者随机分为观察组和对照组,每组各42例。两组均给予常规治疗,在此基础上,对照组采用前列地尔治疗,观察组则采用前列地尔联合百令胶囊进行治疗。连续治疗2周后,观察两组治疗前后24h尿蛋白定量(24hUpro)、血清肌酐(Scr)、血尿素氮(BUN)、尿白蛋白排泄率(UAER)、微球蛋白(β2-MG)等肾功能指标以及白细胞介素-6(IL-6)、白细胞介素-18(IL-18)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)等血清炎性因子水平的变化,并对两组临床疗效进行评价。结果:观察组总有效率明显高于对照组,差异具有统计学意义(P<0.05);治疗后,两组24h Upro、UAER、SCr、BUN、β2-MG等肾功能指标均较治疗前明显降低,且观察组各指标明显低于对照组,差异具有统计学意义(P<0.05);治疗后,两组IL-6、IL-18、TNF-α、hs-CRP水平均较治疗前明显降低,且观察组各指标明显低于对照组,差异具有统计学意义(P<0.05)。结论:前列地尔联合百令胶囊治疗早期糖尿病肾病疗效显著,对患者肾脏具有很好的保护作用,且能有效降低其炎性因子水平,值得临床大力推广和应用。

Objective:To explore the effects of alprostadil combined with Bailing capsule on renal function and serum inflammatory factors of patients with early diabetic nephropathy.Methods:84cases of early diabetic nephropathy patients were randomly divided into the observation group and the control group according to the order of treatment,with 42 cases in each group.oth groups were given conventional treatment,based on which the control group was treated with alprostadil,while the observation group was treated with Alprostadil combining with Bailing capsule.After 2weeks of continuous treatment,24 hurinary protein(24hUpro),serum creatinine(Scr),blood urea nitrogen(BUN),urinary albumin excretion rate(UAER),microglobulin(beta 2-MG)and other indexes of renal function and interleukin-6(IL-6),interleukin-18(IL-18),high sensitive C reactive protein(hs-CRP),tumor necrosis factor alpha(TNF-alpha)changes of serum inflammatory factor levels of two groups were observed before and after treatment,and to evaluate the clinical effects of two groups.Results:The total effectiveness of observation group was 95.23%,significantly higher than 76.19%in the control group(P<0.05).After treatment,24hUpro、UAER、SCr、BUN、β2-MG of two groups were significantly lower than those before treatment(P<0.05),and the observation group showed significantly lower indexes than the control group(P<0.05).After treatment,the levels of IL-6、IL-18、TNF-α、hs-CRP of two groups were significantly lower than those before treatment(P<0.05),and the observation group were significantly lower than the control group(P<0.05).Conclusion:Alprostadil combining with Bailing capsule in the treatment of early diabetic nephropathy curative effect is distinct,it has good protective effect on kidney of patients,and can effectively reduce the level of inflammatory factors,is worth to promote and application.
