目的:探讨急性胰腺炎患者巨噬细胞炎性蛋白-1α(MIP-1α)、巨噬细胞炎性蛋白-1β(MIP-1β)及单核细胞趋化因子蛋白1(MCP-1)的动态变化,分析其在急性胰腺炎病情严重程度评估中的价值。方法:急性胰腺炎患者112例,包括轻症胰腺炎(MAP)组44例、中度重症胰腺炎(MSAP)组36例及重症胰腺炎(SAP)组32例。酶联免疫吸附试验测定患者入院当日、第4天、第7天血清MIP-1α、MIP-1β和MCP-1水平。结果:(1)入院当日MAP、MSAP及SAP组的血清MCP-1、MIP-1β和MCP-1浓度均明显升高,各组间两两比较均有显著性差异(P<0.05)。(2)MSAP组和SAP组血清MIP-1α、MIP-1β和MCP-1浓度在第1天达到峰值,在第4天及第7天逐步下降。MAP组血清MIP-1α、MIP-1β和MCP-1浓度在第4天达到峰值,于第7天降至第1天水平(P>0.05)。各时间监测点血清MIP-1α、MIP-1β和MCP-1浓度均为MSAP大于MAP组,SAP大于MSAP组(P<0.05)。结论:急性胰腺炎患者血清MIP-1α、MIP-1β和MCP-1水平具有一定的动态变化规律,可能对急性胰腺炎病情诊断及治疗提供辅助参考。
Abstract：Objective:To observe dynamic changes of MIP-1α,MIP-1βand MCP-1in Acute pancreatitis patients,and to evaluate the value of MIP-1α,MIP-1βand MCP-1in acute pancreatitis severity assessment.Methods:A total of 112 cases of acute pancreatitis were divided into mild pancreatitis(MAP group,44cases),moderate severe acute pancreatitis group(MSAP group,36cases)and severe acute pancreatitis group(SAP group,32cases).Serum MIP-1α,MIP-1βand MCP-1levels were detected by enzyme-linked immunosorbent assay in patients at 1st,4th day,7th days.Results:On the 1st day,Serum concentrations of MIP-1α,MIP-1βand MCP-1were significantly increased in three groups,with significant differences between the control group,MAP,MSAP and SAP group(P<0.05).Serum concentrations of MIP-1α,MIP-1βand MCP-1reached the peak level on 4th day in MAP group and were significantly higher than that on 1st day.MAP level on 4th day in MAP group was significantly higher than that on 1st day.In the MSAP and SAP group,serum concentration of MIP-1α,MIP-1βand MCP-1reached the peak level on 1st day,and significantly higher than that on the 4th day and 7th day.There was a significant difference between MSAP group and MAP group,SAP group and MSAP group in the serum concentration of MIP-1α,MIP-1βand MCP-1at each monitoring time.Conclusions:MIP-1α,MIP-1βand MCP-1levels have the rule of dynamic change in patients with acute pancreatitis,which are useful in evaluating the severity of the patients with acute pancreatitis.
