目的:探讨老年结、直肠癌合并慢性阻塞性肺疾病(COPD)围手术期处理特点。方法:回顾性分析1997年1月~2007年1月84例老年结、直肠癌合并慢性阻塞性肺疾病患者外科处理的临床资料。结果:84例均伴有不同程度的肺气肿、慢性支气管炎等COPD病史。全组均行手术治疗,术后并发症分别为肺感染10例,腹壁脂肪液化2例,切口感染1例,吻合口瘘1例,呼吸衰竭1例,无围手术期死亡。结论:老年结、直肠癌合并COPD患者,术前加强对COPD的营养支持,肺部炎症控制和围手术期管理,可以明显减少手术并发症,提高手术的安全性。

Objective: To investigate the characteristics of perioperative management of elderly patients with colorectal carcinoma coexisted with chronic obstructive pulmonary disease(COPD).Methods: Clinical data of 84 patients with colorectal carcinoma coexisted with COPD undergoing surgical management from 1997 to 2007 was studied retrospectively.Results: All patients,coexisted with COPD such as emphysema and chronic bronchitis,were treated surgically.Postoperative complications included 13 cases lung infection,2 cases fat liquefaction of abdominal incision,1 case incision infection,1 case fistula of anastomosis and 1 case respiratory failure;no death in perioperative period.Conclusion: Nutritional support,control of lung infection and target management before operation are significant for the safety of surgery and reducing complications in elderly patients with colorectal carcinoma coexisted with COPD.
