目的:总结翼点入路在鞍区肿瘤显微手术的临床应用经验。方法:回顾性分析2005年9月～2008年9月我科经翼点入路对鞍区肿瘤进行显微外科手术的病例42例,讨论鞍区肿瘤经翼点入路显微手术的适应证、并发症,分析影响选择经翼点入路显微外科手术的相关因素。结果:42例患者中,行镜下全切术31例,次全切除术11例,全切率为73.8%。术后并发症:额纹消失1例;尿崩及电解质紊乱18例;无死亡病例。结论:翼点入路为鞍区肿瘤的手术操作创造了良好的手术视野,能较充分显露瘤体及其周围结构,提高鞍区肿瘤的全切除率,为提高患者的生存质量和减少肿瘤的复发创造必要条件。

Objective:To summarize the clinical experience of the microsurgical operation for sellar region tumors via the pterional approach.Methods:The clinical data of 42 patients with sellar region tumors were retrospectively analyzed.All patients were treated by microsurgery via modified transpterional in our hospital inrecent 3 years.The operative resutlts and complications of microsurgery were discussed,and the factors affecting the choice of the pterional approach were analyzed.Results:Of 42 cases,31 had total resection,and 11 cases had subtotal resection,with total resection rate as 73.8%.1 patients had facial paralysis.18 patients had diabetes insipidus and electrolyte disorder.There was no death.Conclusion:The pterional approach for removing sellar region tumors can offer clear operation visual fields,and sufficient exposure of the tumor and it's surrounding structures.It can improve the patients'life quality and decrease the recurrence of the tumor.
